I’ve found that using an embroidery hoop really helps keep your fabric taut, but you can definitely go without if you’re careful. As for thread, I’ve had luck with polyester; it seems to hold up better against wear and tear on canvas than cotton. Also, try a heavier needle to avoid breaking; it makes a huge difference!
